Dear Prospective Patient:

Welcome to the website and services of Lifetime Development!  We appreciate your interest in our work and hope to equip you with necessary information for you to make informed choices about resolving your health and life challenges. 

If you are beginning to research non-medication help for any of the disorders that we treat (focus, memory, anxiety, depression, pain, movement, seizure, learning, dysregulated behavior, balance, autism, movement problems, etc.), you will gain considerable information from the resources below. If you have already made an appointment to have an initial telephone consultation or to see us, kindly complete the forms designated for either "Adult" or "Child" listed above near the top of this page.

There are two required forms: the Background Information form and the Intake History form, either "Child" or "Adult" versions. These need to be supplemented by all related materials you may have that add critical individual information. The greater the detail, clarity, and organization of the information about your health, symptoms, and behaviors of concern we receive, the more efficient our time and your resources will be for reaching a sound understanding of your needs during the initial consultation. Examples of additional materials that are important to us are the following:

educational, hearing, vision, career, and perceptual evaluations

[1] this book was written for parents of autistic spectrum children and may seem irrelevant on the face of it. However the biochemistry described is appropriate to consider in this situation, even though the diagnostic label and many of the symptoms are not shared.
